Summer EBT: Help With PIN, Case Number

DTA put Summer EBT on the SNAP DTA EBT card for families who were getting SNAP/DTA benefits when Summer EBT was issued.

DTA mailed Summer EBT cards to families who were not getting SNAP/DTA benefits when Summer EBT was issued. If you got a Summer EBT card in the mail and are having a hard time activating it (setting up a PIN):

• To active the card, you need to use the case number DTA assigned to your family, along with the date of birth of the person whose name is on the Summer EBT card. 
• The case number is in the upper right hand corner of the notice DTA sent you about Summer EBT. (the notice had a title "Welcome to Summer EBT"). 
• Call 1-800-997-2555 and follow the prompts. When asked, enter the last 4 digits of your case number. 

If you got a Summer EBT card and did not get or do not have the notice and need your case number:

• Call the Summer EBT hotline - 855 425 8770. 
• If you can't get through, fill out this DTA form so that DTA can contact you with your case number. Make sure to enter all information accurately!
