Info for ConnectorCare members who didn't file a 2015 federal tax return

This 2-page flyer is intended for people who received ConnectorCare in 2015 but did not file a federal tax return for 2015. They will not be eligible for ConnectorCare in 2017 unless they file their 2015 return along with a form to account for the advance premium tax credits that are part of ConnectorCare. The flyer explains the situation, where to go for help filing a 2015 return, and the need to inform the Connector after filing a late return in order to get a new eligibility decision for ConnectorCare in 2017.
