Food Stamp/SNAP Work Requirement Medical Report Form: This medical report form can be completed by a wide range of licensed medical, mental health, substance abuse providers to verify that the SNAP applicant or recipient is exempt from the work registration, job search and work rules. Exemptions can be granted if the individual is participating in a vocational rehab, mental health or sustance abuse program; if the individual is pregnant or if the individual suffers from a short or long term illness or disability (lasting 30 days or more) that reduces his or her ability to work.
