NEW Appointment of Representative Form (SSA-1696)

SSA has issued a new SSA from 1696 form. It incorporates changes in the Social Security Protection Act of 2004 providing for non-attorneys eligible to receive direct payment of fees, and a few other things, e.g., hether the representative has ever been disbarred or suspended. Advocates in some areas (not Massachusetts so far) report that SSA has required this new 1696 in ongoing cases.
