DHCD Appeal Notice

Attached is the DHCD version of the appeal notice that gives the numbers for the DHCD hearings division (617)-573-1528 or 1-877-418-3308. Chris McClave is the Acting Hearings Manager . This notice says you can call DHCD recipient services or your local Homeless Coordinator (DHCD worker) to review the file. Recipient Services number is on this notice too (1-877-418-3308). 

WARNING -- as of 12/17/09 we have had reports that the fax number listed on the attached DHCD appeal form (1515) is out of order.  617-573-1285 is apparently the fax number to use instead, but it is advisable to call the docket clerk, 617-573-1528, to confirm.
