Childhood Disability Determination Analysis of Functional Areas Infant (age 0-11 months)

This analysis request an evaluation of the severity of a child's functional limitations in six broad areas of functioning. The information is used to determine whether the child's limitations are functionally equivalent to the impairments listed in the Social Security regulations.
